Volunteer Background Checks

If you would like to serve as a volunteer at your child’s school, you are required to complete the Volunteer Background Check Authorization form each new school year.

Lake Shore Public Schools greatly appreciates the support of our families and the involvement of parents/guardians as volunteers. While we welcome parent/guardian volunteers, it is important that we follow Board Policy 3120.09 (Volunteers) which states that, "Any person who volunteers to work with the District shall be screened through the Internet sites for the Sex Offenders Registry (SOR) list, the Internet Criminal History Access Tool (ICHAT) criminal history records check and the Offender Tracking Information System (OTIS) prior to being allowed to participate in any activity or program." This screening process is to ensure the safety and security of our students and staff in each building.

Volunteers are required to abide by all Board Policies and District Guidelines while on duty. This includes, but is not limited to; following the sign-in procedures established at each building, wearing proper identification, maintaining confidentiality of each student, remaining in a location visible to district staff when working with students, reporting all behavior concerns to the lead teacher and/or building principals, and refraining from disciplining or touching students in a corrective manner.

Volunteers are covered under the District’s liability policy, but are not covered by its health insurance policy nor are they eligible for Workers’ Compensation.

Names of individuals completing a volunteer form will be maintained in a database and shared with building office staff. Buildings may only utilize approved volunteers and will follow their own established processes for contacting approved volunteers throughout the school year.

Process:  

The process for processing volunteers/chaperones follows four steps.  

1. Volunteer completes a background check form each new school year. 

2. The Department of Employee services runs a criminal history check. 
         
3. Teacher submits an event form (below) at least one week prior to a field trip or event where chaperones will be present.
   
4. Building secretary receives the event form and reviews the list to ensure that a volunteer form has been submitted and processed. Teacher will be contacted by the office only if volunteer is not approved.       
   
The Department of Employee Services will contact prospective volunteers only if they are not approved or if additional information is needed to process their form.
